Who Qualifies and Information Needed

 Forsyth County Parents/Guardians
Must live in Forsyth County with children aged up to 18 years* who meet financial guidelines
*(high school aged children, MUST BE enrolled in school)

​Parents/Guardians please bring the following to qualify:

• Photo ID
• Proof of Residency:  Current bill (utility/rent, etc) | Current food       stamps/Medicaid letter
• Proof of Income: Pay stubs OR Current food stamps/Medicaid letter
• Proof of Guardianship: Current Medicaid/Peachcare letter, Birth         certificate or Social Security card

The Place Holiday House is a community event with many people working to make it a success.
There is no guarantee of child/family confidentiality.
